Policies

• Massage Intake form will be sent via email or text message and must be completed at latest day prior to your appointment
• Late arrival for your appointment may shorten or cancel your Massage due to scheduling
• Only one promotional discount can be used per visit
• The massage may be terminated for inappropriate conduct from the client. This includes inappropriate touching, gestures or language on the part of the client. If the Massage is terminated no refund will be given

Medical Waiver & Disclaimer

• Due to Therapeutic Massage services being offered, clients agree to provide any known health issues that may be contraindications for Massage. 
• Those health issues could be, but not limited to diabetes, skin conditions, heart problems and cancer
• Renew and Revive Massage Therapy cannot diagnose any health issues

Privacy

• Any and all client information will be kept confidential 
• Renew and Revive Massage Therapy agrees not to share client information with anyone or entity without client prior written consent
